Lisa Kay Smith

Lisa Kay Smith, age 47, of West Liberty, Ohio passed away at 1:52 P.M. Thursday, December 13, 2018, at Mercy Health in Urbana, Ohio.

She was born in Bellefontaine, Ohio February 8, 1971, a daughter of David “Junior” Smith and the late Sally L. (Chapman) Smith.  In addition to her mother, Lisa was also preceded in death by a grandson Quinton; a sister, Stacey Lynn McCullough; maternal grandfather, Don Chapman; paternal grandfather, John Smith and three uncles, Tim, Todd, and Ted Chapman

Lisa is survived by her father and step-mother, David “Junior” (Diane) Smith; a daughter, Shelby McCullough; a son, Dylan McCullough; a granddaughter, Aniya; a sister, Melissa Ann Williams; maternal grandmother, Ann Chapman Swanson; paternal grandmother, Arlene Smith; two nephews, Landon and Linkon Williams; two aunts, Carolyn (Bill) Roberts and Dawn Chapman and two uncles, Gary (Jane) Smith and Duane (Jackie) Smith.

A homemaker, Lisa was a 1989 graduate of West Liberty-Salem High School. She was a hard worker who loved being with her family, especially her children and grandchildren.

The family will receive friends from 11:30 A.M. to 12:30 P.M. Tuesday, December 18, 2018 at the Eichholtz Daring & Sanford Funeral Home in West Liberty, where the funeral service will begin at 12:30 P.M. with Chaplain Ray Branstiter officiating.

Burial will follow the funeral service at the Fairview Cemetery in West Liberty.

The arrangements are in the care of the staff of the Eichholtz Daring & Sanford Funeral Home in West Liberty.
